This template is used to add a voice actor infobox to a page. To add this template to an article copy and paste the code below and then fill in the information for each section.

|image = The name of the image, followed by its file type, such as Image.png
|caption = The name of the artist who drew the image used in this infobox.
|DOB = The birth date of the voice actor. Enter the following: {{Birth Date and Age|Year of birth|Month of birth|Day of birth|df=y}}
|from = Where the voice actor comes from.
|alias = Any alias(es) this voice actors might use in their work.
|blood_type = The blood type (if available) of the voice actor.
